Participation & Health Disclosure

Supervised Initial Visit: All new clients must be accompanied by a Unify representative during their first, "ice breaker" visit.

Voluntary Participation: participation is voluntary, and you may cease participation at any time if you feel uncomfortable, unwell, or for any other reason

Health Disclosure: you are responsible for disclosing any health conditions that may affect your participation. The evaluations, screenings, and recommendations provided by Unify do not replace medical advice from a doctor or appropriate healthcare provider.

Assumption of Risk & Release of Liability

Risks & Assumptions: participation may involve risks of serious injury, illness, or diability. These risks may result from your own actions and inactions, as well as the actions, negligence, or inactions of others, including Unify staff, and may be related to the conditions of the facilities or equipment.

Voluntary Participation: you voluntarily choose to participate, understanding the associated risks.

Release of Liability: you waive any and all claims for damages, injury, or death that may occur as a result of your participation, and you agree to indemnify and hold harmless Unify Health, LLC., owner Shauna Brown, employees, and volunteers.

Health & Fitness: you confirm that you are in good health and have no conditions that would preclude you from participating in these activities

Legal Validity: this waiver is intended to be as broad and inclusive as permitted by Arizona Law or the jurisdiction in which is executed. If any portion is found to be invalid, the remaining provisions shall continue in full force and effect.

Precautions for Light Therapy

Joovv Red and Infrared light therapy is generally safe, but the following precautions should be observed:

Consult your healthcare provider if you have conditions such as photosensitivity, recent Lasik surgery, pregnancy, or if you are taking photosensitive medications

Discontinue use if you experience any adverse reactions and consult with your healthcare provider if you have concerns about specific health conditions or treatments
